I consent to participate in an orphanage visit in Baja California, Mexico, hosted by the Vida Joven Foundation.

By completing this registration form, I assume all risks which may arise from a trip to another country and an orphanage visit, including but not limited to the negligent, intentional or criminal acts of others, a defect or condition of the premises or transportation vehicle, illness, or an omission of an act of another.

I release and agree to defend, indemnify and hold harmless the Vida Joven Foundation (including their Board, staff, and agents), the orphanage I visit, and the Episcopal Diocese of San Diego and their employees, from any and all claims, causes of action, injuries, illnesses and damages by the participant, his/her/their child and/or guests which arises out of or relates to the trip.

I grant permission to the trip leaders, within their knowledge and ability, to apply limited and appropriate first aid, and/or to obtain medical care for me in the event of an emergency.

I understand that I will bring valid identification for return to the United States. Non-US citizens must bring a valid Green Card, Permanent Resident Card or Passport.

I understand that I am responsible for providing proper documentation to the United States Border Agent for reentry to the USA.
