I hereby assume all risks of my son's  personal injury that may result from the Tokyo Samurai activities. As a parent/guardian, I do hereby release Tokyo Samurai and their instructors and all participants in the Tokyo Samurai Basketball program, from liability, for injury which may result from the student taking part in Tokyo Samurai Basketball activities.
東京サムライの活動に参加している際に生じる可能性のある怪我、事故等のリスクを理解した上、参加します。活動中に生じた怪我その他の事故について、東京サムライ、指導スタッフ、またその他の参加者は責任を負いかねます。

I have given my son permission to participate in the Tokyo Samurai Basketball activities and I certify that he is in good health, has been cleared by a physician and can take part in all physical activities not limited to but including training, practices, and games.  If an injury occurs, I authorize the camp staff members to take all proper action and use the emergency service available at the nearest hospital if necessary.  In case of an emergency, I authorize the personnel to take action.  

By signing this waiver, I also authorize the use of pictures and video of the above-named participant to be possibly used for promotional purposes.
東京サムライの活動に参加するにあたり、参加者が十分な健康状態であり、全ての練習に参加できることを判断の上で当日参加します。もし万が一活動中に怪我が発生した場合は、担当の運営スタッフの指示に従い、必要があれば近隣の病院を利用することを承認します。また、活動中に撮影された写真、動画は今後の東京サムライの活動のプロモーションに使用される可能性がございます。
