Iowa Jazz Championships Copyright Compliance

PLEASE READ THESE INSTRUCTIONS CAREFULLY BEFORE FILLING OUT THE FORM.

For each selection you plan to perform at the Iowa Jazz Championships, include: Title, Composer, Arranger (if applicable), and Publisher. 

Publisher information can usually be found at the bottom of the first page of the score.

If the piece is an original composition that you purchased directly from the composer and there is no publishing company affiliated with the composition, answer "self published" for the "Publisher" question.

If the piece is an UNPUBLISHED CUSTOM ARRANGEMENT of a work not in the public domain (Most compositions after 1929 are not in the public domain, but it is important to check!), you must answer "custom arrangement" for the "Publisher" question AND email sshanley@coe.edu for further instructions to verify the arrangement is copyright compliant. (Note: This situation is not common.)
