Pike Valley Booster Club Funding Request Form
The Pike Valley Booster Club is funded by donations from businesses and individuals in the surrounding communities as well as fundraising efforts of many parents and supporters.

It is our goal to assist with the purchase of items that benefit the students of Pike Valley.  There are limits and guidelines set forth by KSHSAA that we must follow.  Any item purchased by the Booster Club becomes the sole property of USD 426 and must be placed on inventory.  

Use this form to request funding from the Booster Club.  We ask that all requests for funding be submitted TWO WEEKS in advance of need, whenever possible.  Requests will be reviewed and considered by the Pike Valley Booster Club and the building Principal.
