Seating for the recital will be General Admission. Please fill out this form completely. Please plan time for your payment to be mailed, as we will only check the mailbox once on the morning of the performance. The Dance Collective is not responsible for any payments that arrive late or are lost in the mail. Any payments received late will be sent back to the recipient and payment will need to be made at the door the night of the show. Once payment is received, you will be notified via email, and your ticket(s) will be available at The Lebanon Opera House the night of the show.

Adult tickets from the age of 11 are $13.00 if purchased prior to and $14.00 if purchased at the Lebanon Opera House before the show. Children 10 and under tickets will be $10 prior to and $11 the day of the recital

Only check or cash will be accepted for all payments.
