Anyone directly affected by a school matter—including staff, students, parents, guardians, contractors, or community members—may use this form to lodge a complaint.

Please be aware that certain matters are outside the scope of this general complaints policy. Do not use this form for the following issues, which are managed under separate policies:

• Child protection concerns or risks of harm to children.
  
  

• Student bullying or harassment.
  
  

• Student discipline matters, including suspension or expulsion.
  
  

• Student or employee violence or criminal matters.
