Registration: Please fill out all required information and print this form for your records BEFORE YOU CLICK THE SUBMIT button. If you are registering as an individual, please use this first page to register. If you are attending as a team, please scroll down to page 2. (Please note that one person must register for the entire team and will need email addresses for all team members.) If your team is larger than three members, please email a list of all participants including name of participant, email address, and the role each plays (example: teacher, building administrator, full-time mentor, etc.) to firstname.lastname@example.org.
Should you need to cancel, please do so at least three weeks before the applicable training session in order to receive a refund. You will know your registration is confirmed when you receive the following message: "Your information has been entered in my spreadsheet."
