CCES Weekly Meal Sign Up Form- Plan 1
Students attending the in-person program (one-day a week at the start of school) can receive breakfast and lunch for days they are not in attendance.  During attendance plan 1, students will receive 4 breakfasts and 4 lunches to take home. Some items included in the meals will be ready to eat, and others will be heat and serve. Meals will be handed to students as they exit the school by bus, car, or walker. Parents can contact the school cafeteria if they wish to pick up student meals versus sending meals home by student. All meals will be available toward the end of the school day. Chandler Creek is a CEP School and all meals are provided for no charge.
Form may be submitted more than once for multiple students attending Chandler Creek.

Does your student have any food allergies which would prevent some items from being included with the meals? ( for example, if your child is allergic to milk, we would not include) *
Do you understand that meals will be sent home weeky unless you notify the school?  You only need to sign up once to receive meals.  If for some reason, you wish to discontinue the meal program, please send a note to the cafeteria or contact the cafeteria manager at 864-355-2408. *
