Waiver and Release of Liability
I, the undersigned parent or legal guardian of the above-named child, hereby give permission
for my child to participate in the above-referenced school sporting event sponsored by Fort
Smith Public Schools Northside High School..
I understand that participation in any sporting activity carries inherent risks, including but not
limited to, the risk of injury (minor or severe), illness, or death. I voluntarily assume all such risks
and responsibilities.
In consideration of Northside High School allowing my child to participate, I hereby agree to:
1. Release, waive, and discharge Fort Smith Schools, its employees, staff, volunteers,
coaches, and any associated parties from any and all liability, claims, demands, or
causes of action that may arise from my child’s participation in the event.
2. Indemnify and hold harmless the above-mentioned parties from any loss, liability,
damage, or cost incurred due to my child’s participation.
3. Consent to medical treatment: In the event of injury or illness, I authorize school
officials or event staff to obtain any emergency medical treatment deemed necessary for
my child, understanding that reasonable attempts will be made to contact me. *
