To You From Us Youth Participant Release and Waiver of Liability

I, the undersigned parent or legal guardian, on behalf of my child (the “Participant”), desire for the Participant to engage in activities related to To You From Us (TYFU). In consideration for my child's opportunity to participate in TYFU's community programs, I freely and voluntarily execute this Release under the following terms:

RELEASE AND WAIVER OF LIABILITY. I, on behalf of my child, do hereby release and forever discharge, and hold harmless TYFU and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my child’s participation in activities for TYFU. I understand and acknowledge that this Release discharges TYFU from any liability or claim of negligence that my child or I may have against TYFU with respect to any bodily injury, personal injury, illness, death, or property damage that may result from my child’s participation in TYFU's activities.

MEDICAL TREATMENT. Except as otherwise agreed to by TYFU in writing, I do hereby release and forever discharge TYFU from any claim whatsoever that arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with my child’s participation in TYFU's activities.

ASSUMPTION OF RISK. I understand that my child’s participation in TYFU's activities may include a variety of actions, including but not limited to: participating in community events, arts and crafts, community clean-up, and engaging with other participants and volunteers.

INSURANCE. I understand that TYFU does not carry or maintain health, accident, liability, medical, or disability insurance coverage for any Participant or the property of any Participant. I am expected to obtain and maintain my child's own medical or health insurance coverage.

MEDIA RELEASE. I do hereby grant and convey unto TYFU all right, title, and interest in any and all photographic images and video or audio recordings made by TYFU during my child’s participation in TYFU's activities. This includes but is not limited to any royalties, proceeds, or other benefits derived from such photographs or recordings. I further understand that it is my responsibility to remove my child from any area where photos or videos are being taken if I do not wish for my child to be included.

CONFIDENTIALITY. It is the policy of TYFU that participants may not disclose, divulge, or make accessible confidential information belonging to, or obtained through their affiliation with TYFU.

OTHER. I expressly agree that this Release is intended to be as broad and inclusive as permitted by the laws of the State of Texas, and that this Release shall be governed by and interpreted in accordance with the laws of the State of Texas. I agree that in the event that any clause or provision of this Release shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release which shall continue to be enforceable.

Acknowledgment and Agreement

By providing my name and the date below, I acknowledge that I have read and understand this Release and Waiver of Liability and agree to its terms on behalf of my child. My entry of this information serves as my legal signature.
