Welcome to the SEED (Search-Embark-Experience-Develop) 2016 Annual Retreat Registration.

The entire registration process is online (with the exception of the payment, of course, which still must be sent via post mail).  Having an online automated process allows us more time and energy to focus on the goal of this retreat: Your Child.

A few VERY IMPORTANT changes happening this year, with regards to both the registration process as well as the Retreat itself.  Please read and follow instructions very carefully!.  By not completing requirements your registration will become invalidated.

IMPORTANT:  One very important addition this year is the requirement for the Survey, which is very critical for the SEED Leaders in their development of the Retreat Program.  At the end of this registration (and also in the Confirmation email) there will be information on how and when the Survey needs to be completed. PARTICIPATION AGREEMENTS
The agreements in this section are applicable for all of the children (retreatants) that are attending this retreat.
VIDEO AND PHOTOGRAPH AGREEMENT
I understand that any photographs or videos taken of myself and/or my child during SEED Retreat may be used on Dong-Hanh CLC websites and publications. Should I not want the picture or video used in any way, I will inform Dong-Hanh CLC, who is the SEED Retreat organizer, in writing, before the retreat.
RELEASE AND WAIVER OF LIABILITY AGREEMENT *
Dong-Hanh CLC is not responsible for lost and/or damaged personal property. I also agree to release, waive, discharge, not to sue, and hold Dong-Hanh CLC, its directors, officers, organizers, and volunteers harmless from liability on account for any loss, or damage, or claim, or demands or any injury to the person including death. Any claims for injuries shall be limited to the insurance coverage available by Dong-Hanh CLC for the SEED Retreat purposes. I further expressly agree that the foregoing ‘Participation Agreement’ and ‘Release and Waiver of Liability’ are intended to be as broad and inclusive as is permitted by the law of the State of California and that, if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. IF YOU DO NOT AGREE WITH THIS AGREEMENT (THE BOX BELOW IS NOT CHECKED), YOUR REGISTRATION CANNOT BE ACCEPTED.
ACKNOWLEDGEMENT OF AGREEMENTS *

IMPORTANT:  To ensure your reservation, please mail the payment to the above address as soon as possible. Please note that your registration will not be completed until the payment has cleared. A Welcome Package will be sent to you via email to confirm your registration. We will also notify you via email if you are on the waiting list.

Please contact us, if two weeks after sending in the check, you receive neither the confirmation email, nor the waiting list number.
