Non-Emergency Community Support Request Form

Cambridge Heart is actively accepting non-emergency requests. We handle each inquiry with care and consideration. We have a limited capacity as we continue to build our organization, but we look forward to growing with our community. We have designed this request form based on our capacity; each request is reviewed individually by the HEART team. Then, they are assigned one primary responder and at least two secondary responders with support from fellow members. 


HEART strives to carry many values into our relationship with the community, to name a few: Self-Determination, Mutuality, Confidentiality, Healing, and Self-Care. We work as a collective with respect to all of these values to help provide any person with the tools they need to move forward.
