This form provides us with vital information to apply for grants so we can continue to provide diapers. Rest assured, your information is protected, and we will never sell your information to anyone. 

In addition to diapers, we offer gently used children's clothing to agencies we are in contact with. All clothes are laundered before donation. Unless otherwise specified, we will provide clothing seasonally: long sleeve shirts, pants, sweaters and coats from mid-September to February and t-shirts, shorts, dresses, and swimsuits (if available) from March to September. If the child's shoe size is provided we will also include gently worn shoes, when available. 

You may include up to three children per form, and fill out as many forms as needed.
