SMART Registration Form for Term 3, 2021

Dear Parents

SMART is now taking enrolments for Term 3 2021, starting on Monday 2nd August.

Please complete the registration form as soon as possible to confirm that your child will be attending SMART during Term 3.

Terms and Conditions of St Mark's Church School SMART Programme
Terms and Conditions for St Mark's After School Care Programme - SMART (as per enrolment forms)

* Opening hours are 3:05pm - 5:30pm
* A one off, hard copy enrolment form must be completed if this is your first time using SMART, as well as the registration online each term
* Fees:
    - $22.75 per session for full session (for regular bookings and casual bookings in advanced)
    - $28 if you book on the day (please text, phone or email  to check availability)
    - $1 per minute late fee after 5:30pm
(Please note we have removed the early pickup option due to administration reasons. We will still accept enrolments if you have other school activities).

* Absences:
 If your child is absent and
     - you notify SMART before the day you will not be charged
     - you notify SMART on the day you will be still charged $22.75
     - you don't notify SMART you will be charged the full fee ( please note you must text, email or call SMART directly)

* Fees will be charged to your St Mark's sundry account each month
* Only recognised people may pick up your child
* If your child does not turn up to SMART when they are expected we will endeavour to contact you to confirm their whereabouts
* It is a legal requirement for you to sign your child out each day.  No students are able to do this.
* Students in the homework programme (Year 3 - 8) are required to bring their homework, a pencil and a book to read, to SMART each day.
