Community Health Contact Form

Ways of reaching us

This form is a way you can contact us that allows you to remain anonymous, and allows us to ask structured questions.

Please note that in order to remain anonymous, you should take care not to enter personal details anywhere.
Here, you can also choose to speak to someone in person. You can also directly reach out to our volunteer point of contact Alix Pham (former EACH Co-Director), or our team.

You might contact us here because you want to…

  • Talk through a difficult personal issue and/or find support regarding your mental health
  • Talk through a complicated situation with another person/people (that you are involved in or you are witnessing)
  • Report something you experienced in the EA community which made you feel uncomfortable in any way, or something that is making It difficult for you to participate in the group
  • Report a seemingly unpleasant or stressful situation that you have observed
  • Let us know if you suspect/have witnessed someone behaving in a way that’s likely to cause harm (behavior that disturbs group meetings, harassment, mean behavior, dishonesty, etc)
  • Ask us to pass on feedback to an EA organization or organization employee on your behalf
  • Notify us of a concern about trends within the effective altruism (or an associated) community
  • Anything else that you think we ought to know and/or do something about
If you are unsure if your experience or observation falls under one of these categories and is worth reporting, we encourage you to err on the side of disclosing. We prefer to alleviate concerns early, rather than learn of problems too late.

If you want to give feedback for our work as the EACH team, please use this form. You can also contact the Community Health team at the Centre for Effective Altruism by using this form.

Confidentiality

All reports given to us will be kept confidential by default. You will be able to decide what happens with your report. Nevertheless, we would like to advise you that, despite our efforts to maintain confidentiality, it can happen that witnesses are identified.

This form gets checked by Alix Pham (vacation replacement: Marcel Steimke). We try to answer requests for contact within three days max. If both of us are out of office, submissions might get checked by another team member. If you have a strong reason for withholding information from a particular EACH Team member, please contact one of us personally (e.g. on Slack).

If you have further questions about how we handle confidential information, we’re happy to discuss that at the start of our conversation.

What would you like to share with us? *
If you want to report a misconduct by someone within / aligned with the EA community, please also fill in the detailed questions in section 2, if you feel up to that.
How would you like us to follow up with this, if at all?
Feel free to share your ideas on how we should respond. You can also tell us here if your contribution is only for our information.
If you wish to be contacted by us, we will have a conversation to clarify possible steps, and take further action if necessary (e.g. if another person is involved).
How urgent is this concern? *
Not urgent at all
I would like a response within 2-3 days
If possible, please explain why this concern is more or less urgent for you. A short sentence is enough.
Would you like to get in touch with Alix, our Community Health Contact? Then you can book a slot in her calendar, or find a different time via email.
*
Or is there someone else you would like to talk to? You can mention it by selecting "Other...".
Have you already spoken to someone within the EACH Team or the Centre for Effective Altruism Community Health Team?
Or anyone else that could be relevant.
Reporting Misconduct
If you didn't report a misconduct, you can skip these questions.

These questions help us get more details about the incident(s) you want to report, which might make it easier for us to take action and evaluate possible actions.
What kind of violation do you want to report?
How is the person connected to the EA Community?
For example, has the person attended a retreat or local events?
Please note the place, date and time, if possible, where and when the incident took place.
If you're reporting several instances, please enter a time range and note any specific instances in the narrative.
Were bystanders present when the incident(s) occurred?
If so, please tell us if you know them and if we can contact them. If possible and sensible, please give their name(s) and contact details. We won't contact anyone without discussing it with you first.
Contact details
You can submit this report anonymously. If you do, you should not wish to have any follow up contact.
Please bear in mind that anonymous information is sometimes of limited value, e.g. if we have no way of contacting you to ask follow-up questions.
(Optional) Your name and pronouns
(Optional) Preferred contact details
Phone number, messenger contact, email, etc.
(Optional) If you're filling out this form anonymously, you're welcome to give yourself a number or pseudonym here that you can also use for reporting future concerns.
This would allow us to track which concerns are coming from the same person without breaking their anonymity, and might be useful for you if you think you could end up reporting multiple concerns of a similar nature.
How confidential would you like us to keep this? *
For example: Could we share a full version of this with people we think might be able to help you? Someone who seems to be in a similar situation? Or someone who needs to make a decision that this information could be important for? Would you like us to follow up with anyone else involved?

This is most important if you’re contacting us anonymously - if you’d like to talk further with one of us, we can work this out later.
