The Sons of Thunder are introducing a Paperless Liability Waiver System. This system will benefit our game patrons with quicker Waiver check-ins on game days. It will also benefit SOT with reduced paper consumption and storage. The Paperless Liability Waiver System will also allow for quicker completion of Liability Waivers at games. Those enrolled in the Paperless System will simply: Arrive, Provide Their Name (Last, First) to the Liability Waiver Attendant, and Sign and Date on the Tablet.

By filling out this form, you will be added to the SOT Paperless Liability Waiver System.  For new submissions entries must be received a week before a game day for availability on that game day. This form must be filled out once per person participating, one form does not cover both parent and minor, or siblings, one form is required per person.
