Youth Trip Code of Conduct & Disclaimer Signature Form

I have read and understand and agree to the standards found on the PSM website or the packet for conduct. I understand and agree that in the event that my conduct is considered so unsatisfactory that it jeopardizes the success of the trip, and that mediation during the trip has failed to correct my behavior, that my services with the trip shall end and I shall return home immediately at my own expense.
