OHES Student ID/Lanyard Replacement Request

This form is to request a replacement order for your child's OHES Student ID and/or Lanyard.
Please note, once this form is submitted, a $10 fine will be generated in Genesis that must be paid before receiving a replacement. No parts will be given before payment is received.
By submitting this form you agree to be charged for the new item, even if you later find the lost item, so kindly do your due diligence to search for and locate your missing ID/Lanyard before submission.
Please allow 1-2 school days after submission & payment is complete to receive replacement.
As per new school guidelines, all students and staff must visibly wear their IDs on the school authorized lanyard at designated times in the building and on school grounds.
