Merryhills Breakfast & Teatime Club Registration Form
This form must be completed in full and submitted as part of your child's registration onto Breakfast and/or Teatime Club.  Once the form has been submitted, please allow 48 hours for access to be set up on Arbor.

One form is required per child who will attend.

Bookings must be made 24hrs in advance. If possible we will try to accommodate emergency bookings but due to staffing and childcare ratios this may not always be possible.

Parent agreement
I am aware that Breakfast Club opens at 7.45am and latest collection from Teatime Club is 5.45pm

I understand that there is a charge of £1 per minute if my child is collected from Teatime Club after 5.45pm

My child will abide by the school rules and behaviour policy whilst attending Breakfast and/or Teatime Club

I have read and am aware of the school's Charging policy (available on www.merryhills.net), that all booked sessions must be paid for and no refunds will be given for non-attendance.

I consent to the provision of any emergency treatment for my child if necessary during Breakfast  and/or Teatime Club

Should it be come necessary to take my child to hospital I authorise a member of staff from Breakfast and/or Teatime Club to sign any written form required by hospital authorities if a delay is considered to endanger my child's health and safety by a doctor.

I understand that every effort will be made to contact me to obtain consent.
