Thank you for your interest in the MHCC chaperone service. Please fill out one form for each child, at least one week in advance, so that we are able to properly staff Kids Club. It may be easier to fill this form out once, to account for the entire summer, but it may also be filled out week to week. You will receive a copy of your responses upon completion. Reservations for the Chaperone Service must be made ONE WEEK IN ADVANCE from the week you will be utilizing this service. This helps us tremendously in ensuring we can provide the appropriate number of staff members to keep your child(ren) safe while they are with us at the Club.
This Service is billed at $75/week. For your Child to be eligible s/he must participate in at least 2 different sport clinics per week and at least 1 per day (any combination of golf, tennis, swim team/lessons). Any day your child does not particiapte in a sport clinic will be billed separetley at the regular Kids Club billing rate.
When filling out this form, please be as detailed as possible about your childrens schedule for each day including drop off/pick up times, which clinics they are participating in on which days, and when they should eat lunch. You are welcome to pack their lunch or fill out a food order form daily to be ordered from the Snack Bar.
NEW THIS YEAR!!!--- Swim Hours Wednesdays 10a-11a, Friday 1230p - 130p
--- This year we will facilitate free swim hours where your children can enjoy free play at the pool. Our staff will monitor for the pool deck so please pack any floaties/life-jackets and proper swim attire. This is an optional activity for kids and is not required.
We will begin accepting morning Drop-Offs at 8am and the Chaperone Service will conclude at 3:00pm daily. Hours outside of this time frame will be billed at the regular Kids Club rates.
We will accommodate transporting your child to afternoon swim team practice if requested (requests can be made by noting it in your child's daily schedule on this form).
PLEASE NOTE: If you request this accommodation, a parent or guardian MUST be present at the conclusion of Swim Practice to pick up your child. Please check in at the Pool Desk so Club Staff is aware you are present.
LATE PICKUPS WILL BE BILLED AT $1/minute for this accommodation.
