1. Complete ISA Zakat Form clearly to the best of your knowledge.

2. Provide complete income information of the household (income of all members including husband, wife and children), as well as social security income, compensation of any kids, child support, low income housing, TANIF, WIC, food stamps, and any Financial help from other Masjid or any other organizations.

(ISA will request the following information as needed)

1. Provide current Maryland ID (driver’s license or state-issued identification card)

2. If MARRIED, PROVIDE THE FULL NAME AND VALID IDENTIFICATION OF YOUR SPOUSE.

3. Provide, if asked IRS fillings and / or letter describing your future financial plan.

4. Provide a statement or termination letter if you claim to be unemployed.

5. Provide copies of supporting documents; for example, lease agreement for rent, or gas, water or recommendation from an imam of the masjid or center in your area.

6. If you live outside the “ISA” area (where “ISA” holds Friday prayer), provide a letter of recommendation from an imam of the Masjid or center in your area.
