I/We, the parents/guardians of the above-named Baseball U LI candidate, hereby give my/our approval for their participation in any and all organization activities.

Athletic Injury & Liability Release I/We recognize that participation in competitive baseball involves inherent risks of serious injury. I/We understand that protective equipment does not prevent all injuries to players. By signing below, I/we hereby waive, release, absolve, indemnify, and agree to hold harmless the Merillon Sports Association, Baseball U LI, their organizers, sponsors, supervisors, coaches, participants, and any persons transporting my/our child to and from activities, from any and all claims arising out of any injury or illness to my/our child, whether the result of negligence or any other cause.

Media & Digital Release I/We understand and agree that photographs or video footage of my child taken during tryouts or organization events may be posted on digital media platforms, including the official website, Twitter/X, and Instagram, for organizational and promotional purposes.

Participant & Parent Responsibility By agreeing to this waiver, I/my child accept full responsibility for the actions of the participant. I/my child understand that we are solely responsible for knowing and adhering to all Baseball U LI rules, regulations, and event policies.

I/my child also acknowledge that it is our responsibility to provide workout officials with an accurate, active parental cell phone number and email address for critical communication (e.g., weather updates, schedule changes, or emergencies). I/my child understand that the participant is expected to be fully available for all scheduled tryout events and that all required registration paperwork must be completed and turned in by the dates set forth by Baseball U LI.
