Artist Submission Form for Display in Commercial and Office Spaces (Milton and Possibly Halton Region)

Are you interested in showcasing your artwork in commercial or office spaces around Milton—and potentially in the wider Halton area? If so, please complete this form.

Artists will receive a monthly rental fee for each piece displayed. If a piece is sold, the total rent received up to that point will be deducted from the sale price.

Example:
Your artwork is priced at $500. It is rented out at $50 per month.
FASM takes a 20% commission, so you receive $40/month.
If the artwork is sold in month 3, you will have already received $120 in rent.
At the time of sale, you will receive the remaining $380 ($500 - $120).
In this case, the client purchasing your artwork will only pay the final sale price; no rental fee will apply.

Note : Incase of external purchase by customer client then this will be processed as a standard FASM sale. You will receive the Sales Price minus a 20% commission to FASM.

Instructions:

Please complete the form as thoroughly as possible.
If any of your artworks sell, notify FASM immediately.
To submit additional artworks, please fill out a new form for each batch.
