WY SNA Conference: School Food Service Employee of the Year 2017-2018
Please fill out all of the requested information for the individual you are nominating for the "School Food Service Employee of the Year". This award is intended to recognize the value and contributions of school food service employees to their district/school. Nominations can be submitted by school food service staff, school administration, secretaries, teachers, food service directors, etc. All award entries are confidential. Please avoid using the nominee's name and school/district in your responses (except the first question).

- Any school food service employee except the food service director (cooks, cashiers, managers, dishwashers, secretaries, etc)
- Does NOT have to be a member of the National or Wyoming School Nutrition Association
