2023-2024 PPS K-5 Health Exemption Form
Dear Parent/Caregiver,

With the change in the NJ Health Curriculum, there are several sensitive topics that will be taught at the end of Grade 2 and Grade 5.  In Grade 2, students will learn the definition of reproduction and discuss how parents take care of their offspring.  In Grade 5, students will learn about gender identity versus sexual orientation, the changes their body goes through during puberty, and human reproduction.

Please ONLY complete the form below if you are requesting that your child be EXEMPT from one or more of the lessons listed below. 

If you have multiple children in our K-5 schools and would like them to be exempt, please complete this form once for each child for the 2023-24 school year.
