Adoption Questionnaire
DUE TO CURRENT COVID-19 CONCERNS, AAC HAS HAD TO ALTER THE ADOPTION PROCESS. PLEASE FILL OUT THE QUESTIONNAIRE BELOW. THIS MUST BE COMPLETED BEFORE YOU COME TO THE SHELTER. IF WE DON'T HAVE ONE ON FILE FOR YOU, YOU WILL BE ASKED TO STEP OUTSIDE AND FILL ONE OUT.
Our adoption questionnaire is not binding in any way, nor does it reserve or "hold" a dog. Our responsibility is to find the best homes possible for each and every one of our great pets and this questionnaire is the first step in that process. That being said, please fill this out to the best of your ability. There's no such thing as too much information.
