Mum Fit Wonder PAR-Q Form
The Mum Fit Wonder Physical Activity Readiness Questionnaire is designed to determine your safety when taking on exercise whether online or in-person. Most people can start an exercise plan or routine without worry; however, some people may require a preliminary health check to ensure that they are taking on the right level of activity for their medical condition.
A PAR-Q highlights any medical conditions or physical inabilities that a client may need to speak to their GP about and that Lauren or any other trainer may need to take into consideration during training. From a liability point of view, the PAR-Q reduces the risk of client injury, illness and even accident.

Has your doctor ever said that you have a heart condition and recommended
only medically supervised physical activity?

If you answered YES to one or more questions: 
Talk with your doctor by phone or in person BEFORE you start becoming much more physically active or BEFORE you have a fitness appraisal. Tell your doctor about the PAR-Q and which questions you answered YES. You may be able to do any activity you want — as long as you start slowly and build up gradually. Or, you may need to restrict your activities to those which are safe for you. Talk with your doctor about the kinds of activities you wish to participate in and follow his/her advice.
 
If you answered NO to all questions above, it gives a general indication that you may participate in physical and aerobic fitness activites. The fact that you answered NO to the above questions, is no guarantee that you will have a normal response to exercise.
