TERMS & CONDITIONS
The City of Philadelphia through PHLConnectED is providing you with this WiFi-capable Internet Hotspot (“Hotspot”) along with up to (24) months of prepaid high-speed Internet access (“Service”) or through June 30th, 2022 from T-Mobile USA, Inc. (“T-Mobile”), for the singular purpose of allowing your student(s) to participate in remote learning and out-of-school instructional activities, as long as your student(s) remain(s) actively enrolled in a K-12 school and a resident of Philadelphia. Your use of the Hotspot and Service is subject to the Acceptable Use Policy of the school that is distributing your hotspot. Please ask your school for their policy, if you do not know where to find it.
You acknowledge by accepting this Hotspot and Service from T-Mobile, that you assume all responsibility for its care and use. The City of Philadelphia shall transfer ownership of the Hotspot and the associated Service to you directly, and shall assume no legal, financial or other responsibility for its use or misuse. Accordingly, you consent to allow the City of Philadelphia to provide T-Mobile with the name of your student, their parent and/or guardian, and their basic demographic information, in order to initiate the transfer of ownership of the Hotspot and Service from the City of Philadelphia, to you. Hotspot and Service is intended only for your student or for the student household identified above and is not transferable. You acknowledge that the City of Philadelphia reserves the right to end the PHLConnectED program, discontinue Service, or recall the Hotspots at any time.
Lost, stolen or damaged Hotspot equipment, overage fees, or other financial responsibility arising from the use or ownership of the Hotspot or Service shall be solely your responsibility. Under no circumstance will the City of Philadelphia purchase replacement Hotspot equipment. Further, the City of Philadelphia makes no guarantees or representations as to the performance, geographic coverage or data usage limitations of the Hotspot or Service provided by T-Mobile. You are advised to refrain from use of the Hotspot for purposes other than school-related activities, such as games, streaming movies and music, or other uses that may quickly affect hotspot performance required for online school work from T-Mobile. T-Mobile reserves the right to utilize and to enforce the use of Internet filtering or other technologies that may limit access to certain Internet sites that are deemed inappropriate or that otherwise violate their terms of service.

In the event that your student graduates or is no longer a resident of Philadelphia prior to June 30, 2022, you must return the Hotspot device to your student’s previous school. If by June 30, 2022, you wish to continue to use your T-Mobile Hotspot, you have the option to bring your Hotspot to any T-Mobile retail store to arrange for the purchase of a new Internet service plan, solely at your cost.. If you take no further action after June 30, 2022, your wireless Internet service plan will expire and the Hotspot will no longer function.
