Guest acknowledges, agrees and understands that weight training, aerobics and cardiovascular training can be hazardous to some individuals and may result in injury to guest or other members. Guest further understands that all exercises including the use of weights, number of repetitions, and use of any and all machinery, equipment, and apparatus designed for exercises shall be at Guest’s sole risk. Notwithstanding any consultation on exercise programs, which may be provided by FIGHT OR QUIT GYM, or its principals, owners, employees, agents, and/or sub-contractors, it is hereby understood that the selection of exercise programs, methods, and types of equipment shall be Guest’s entire responsibility. Guest further agrees that in consideration for permission to enter onto the premises of FIGHT OR QUIT GYM, guest assumes all risks of injury incurred or suffered while on and/or upon the premises of FIGHT OR QUIT GYM, and releases and agrees not to sue FIGHT OR QUIT GYM, its principals, owners, agents, servants, associations, employees or anyone connected with FIGHT OR QUIT GYM for any claim, damages, cost or cause of action which guest has or may have in the future as a result of injuries or damages sustained or incurred while on and/or upon the premises of FIGHT OR QUIT GYM.

CODE OF CONDUCT
We politely and respectfully request that you adhere to the following guidelines during your tenure at Fight or Quit Gym. 1. Respect the facility, equipment, and other members at all times.2. Please be mindful of your surroundings, others, and use common sense. When someone is setting up for a lift, doNOT walk in front of him or her.3. Do NOT crush the blocks of chalk.4. Do NOT put anything on the upholstery (beverages or weights). Water bottles stain and weights stretch out the vinyl.5. Safeties in the combo racks must be set at the correct height for bench at all times. Safeties in the power racks must always be set at the correct height when squatting. Barring a catastrophic injury, barbells should never be dumped off the back onto the safeties.6. Calibrated KG plates shall ONLY be used for squat bench and deadlift NOT on any machines or for auxiliary lifts.7. SQ and BP attempts over 90% require spotters at all times.**I understand that I will be financially liable for damage to equipment due to negligence. This includes but is not limited to barbells and racks. 8. When combo racks are not in use the bench goes back and bar is placed on bench cradles.9. After use, ALWAYS wipe the head of the benches with the spray bottle and paper towels provided. (Failure to do so ruins the vinyl and makes the benches slippery).10. LAST PERSON TO LEAVE THE FACILITY: The last person to leave the facility must shut off the lights and swamp coolers. Be sure the door is locked and completely closed behind you.11. Guests must sign both a guest waiver and code of conduct before working out. In addition guests are subject to a $15 fee (day-pass). I affirm and agree that I have read the above statements, understand them completely, and agree to abide by all rules and regulations during my tenure atFight or Quit Gym. I understand that management has the right to suspend and/or terminate my membership for failure to observe these rules and/or behavior inimical to the enjoyment of the club by other members.
