This form is required for some courses offered through Prosper ISD CTE that may require time outside of the classroom. The PISD Middle School course catalog contains information about grade level, credits, course descriptions, and prerequisite requirements. Because of site limitations and program seat availability, simply selecting the course and filling out the form does not guarantee actual enrollment in the desired course(s) for the upcoming school year. This form must be completed in entirety. A form must be completed for each course in which you wish to enroll.
