I have read the camp application form and agree to the terms and rates stated on the website.   I give my child permission to attend all trips.   I understand that in any medical situation every effort will be made to reach a parent/guardian. If for any reason I cannot be reached, I hereby authorize M-Cat Sports Camp at Maimonides School to transport my child to the nearest hospital for the necessary medical treatment.  M-Cat Sports Camp at Maimonides School is hereby granted the right to reproduce electronic or printed images of its campers as part of any camp-related activity. If there are questions relating to this matter they may be submitted via email to mcatsportscamp@maimonides.org.  I hereby certify that my child is healthy and I will notify camp in writing of any allergies or conditions that he or she may have. I understand that I must submit a physician's physical examination form including my child’s immunization history. Payment Information
Maimonides faculty receive a 10% discount. A non-refundable tuition deposit of $200 per camper will be billed to your Smart Tuition account upon submission of the Registration Form. If registering before March 20, 2020, the balance of the “early bird” tuition after deposit will be billed to your Smart Tuition account for payment in April 2020. If registering after March 20, 2020, the balance of the “standard rate” tuition after deposit will be billed to your Smart Tuition account on the next available payment date but no later than June 1, 2020. *
