Chaperone Request Application - SHHS Orlando 2026
This form is to apply to be a chaperone on the Science Hill High School Orchestra competition trip to Orlando, Fl. This is only an application. This is not a guarantee that you will be chosen to be a chaperone on this trip. Chaperone applications will be reviewed and decided on by the orchestra director and the orchestra guild board members.

We will be departing from SHHS the morning of Thursday, March 11 and returning the evening of Sunday, March 15. This trip will include various planned activities including 3 days at Universal Studios amusement parks. We are scheduled to compete on Saturday , March 14. Chaperone cost is $790
Applications received after November 15th, 2025 will not be considered unless other written notice was received by the guild or director.

  • A request to be a chaperone must be submitted and approved by the orchestra director and guild board prior to making a deposit and being accepted to attend the trip.
  • Chaperones are financially responsible to pay the chaperone trip cost to the guild adhering to the same guidelines as the Trip Contract outlines.
  • Chaperones will be expected to attend a Chaperone Meeting before the trip if the guild deems necessary. This will be scheduled the week before the trip. Every effort will be made to accommodate a date and time suitable for ALL as ALL CHAPERONES WILL NEED TO ATTEND.
  • All chaperones will remain with the orchestra at all times to assist with performance preparations, to offer support and encouragement, and to provide attention to the orchestra’s specific needs.
  • After a performance, chaperones will stand by and wait for specific instructions from the director regarding what is to happen next (i.e.: reload bus, watch other performances, mealtime, etc.). Always remain with the students unless otherwise instructed by the director.
  • Keep your group together and in sight at all times.
  • Keep students on schedule.
  • Notify a teacher immediately if any problems arise.
  • Refrain from the use of tobacco or alcohol products in any form, at any time, during the school trip. Wherever students are is considered “school property”.
  • Wear appropriate attire that adheres to the school dress code.
  • Abide by all school rules while on this trip.
  • Serve as a role model for students and let your conduct reflect this at all times.
  • Siblings and other guests are not allowed on field trips.
  • Per the Johnson City Schools Volunteers/Chaperone Requirement policy, all volunteer chaperones, which includes parents/guardians, who attend field trips must be registered through Raptor at the school site and not have any criminal convictions. Volunteers/Chaperones that attend overnight field trips are required to submit to a TBI background check through IdentoGo fingerprinting service and receive a favorable report before departure.
