Open House Waiver & Consent
By registering for a Dance Envy Riverview Open House class, I acknowledge that participation in dance and physical activity involves inherent risks of injury. I voluntarily choose to participate (or allow my child to participate) and assume all associated risks.
I release and hold harmless Dance Envy, its owners, instructors, staff, and volunteers from any claims, injuries, losses, or damages arising from participation in these activities, except where caused by gross negligence or intentional misconduct.
I confirm that I (or my child) am physically able to participate. I also give permission for Dance Envy to take photos and/or videos during the Open House for promotional purposes unless I notify the studio in writing before the class.
