Permission to Participate, Photo & Medical Release Form

I hereby grant my permission for the above named child (the Participant) to participate at Camps for Kids, conducted by Calvary Baptist Church. I give permission for Calvary to photograph and video tape my child as a part of regular program activities and allow any photos or video that includes my child to be published. Further, I hereby grant permission for any adult staff member of said program to seek and authorize emergency medical attention on behalf of my child in my absence and hereby release Calvary Baptist from any liability associated therewith. Signing my name will serve as my electronic signature, and I represent that I am the parent or legal guardian of the participating child/children.
