ALEXANDRA'S PLAYGROUND is committed to bringing children the opportunity for safe and active play, especially in areas where they wouldn't otherwise have them. We are a NY based 501c3 nonprofit. When we donate a playground the Site Owner will take ownership of the playground and be responsible to maintain the donated playground and its safety surface.  alexandrasplayground.org

Alexandra's Playgrounds must be open to the public every day, and must be in an easily accessible and publicly visible location. Briefly describe the site you propose. *
Tell us the expected hours the playground will be open to the public. (If the site is a school, the playground can be open to the public after school hours and on weekends. For school sites indicate the days and times when the playground will be used by students, and the times open to the public.) *
How many children will be using this playground? If a school, include the number of enrolled students this playground will be designed for, and separately indicate the number you expect from the neighborhood. If after school or weekend programs are run at the site include information on them - days and hours the program runs, and enrollment information. *

Alexandra's Playground aims to fill a need for safe and active play in underserved neighborhoods. Describe locations of the nearest playground(s) open to the public (including schools). Include site(s) name and address, and any other pertinent information.  *
Describe the economic conditions in this community. Include whatever data you may have: percentage of residents living below the poverty line,  percentage of students on free or reduced lunch, etc. Please include as much detail as possible. *
An Alexandra's Playground donation includes Engineered Wood Fiber safety surfacing. If rubber or other safety surfacing is preferred then the Site Owner will need to provide and install the material and prepare the sub-surface as needed. What is the expected safety surface? *
When Alexandra's Playground donates a playground with Engineered Wood Fiber (EWF) safety surfacing, the site owner is responsible for providing a level site, cleared, with no obstructions, and ready to receive the EWF. Preparation by Site Owner may include removing asphalt and installing drainage, or installing rubber pads designed for use with EWF. If Site Owner will provide an alternate safety surfacing they will need to prepare the site as needed. The site owner must agree to provide the necessary drawings and building permits (Including all associated costs).  Is the site owner or their representative able to do this? *
Will the site owner agree to the ongoing ownership and maintenance of a new playground, and maintenance of the playground safety surface? *
We prefer our playgrounds be built under a 'Community Build' model, whenever possible. Are you or another community member available to gather and coordinate with local volunteer community members to participate in the build? *
Please describe the part of the site where you would like the playground to be installed, especially if the site is large enough to consider multiple locations. (indicate cross streets or other identifying characteristics) *
If request is for funding as part of a larger overall project: describe the project, list all major funders and supporters of said project. Submit related documents to us via email.
