Use the form below to anonymously report an incident or a crime. All information is kept confidential and private by the school to the extent allowed by law. No information regarding the sender (name, email, IP address, etc.) will be collected when submitting information to administration through this page. Please be as complete as possible.

If you do wish to have administration contact you regarding this incident, please include contact information below.

NOTE: If you need IMMEDIATE ASSISTANCE call 911. This form generates a report that is normally reviewed during business hours and should not be used if you need an immediate response.

This form is not intended to replace 911 in the case of an emergency.
