Bella Terra Morton Grove Visitation Request
Bella Terra Morton Grove is working with our local and state public health departments in order to establish indoor and/or outdoor visits between our residents and their loved ones. With that being said, we are excited to help arrange an indoor and/or outdoor visit with you and your loved one, when they become possible.

If you are interested in signing up for an indoor and/or outdoor visit with your loved one, please read the important guidelines below, and then complete the below with your information. These guidelines were established to safeguard every resident to the best degree possible and mitigate the exposure of our residents, staff and visitors to the COVID-19 virus.

After completing this online form and the facility has finalized all preparation, a staff member will reach out to you to help arrange a visit with your loved one.

Please read the fact sheet below, non-compliance to the fact sheet will result in the visitation getting cut short.

IMPORTANT GUIDELINES

General:
•Visits are by appointment only.

Indoor guidelines:
• Visiting hours are between 10:30 a.m., and 2 p.m. and 6 p.m.
• Outdoor visits are strongly encouraged, weather permitting, and are strongly preferred over indoor visits.
• Subject to IDPH and CMS exceptions, indoor visits are only allowed in facility-designated common areas and not in a resident’s room.
- However, if an IDPH or CMS exception applies, visits may take place in a resident’s room, subject to IDPH and CMS guidance.
• Visits will be limited to 30 minutes per visit, due to capacity required for social distancing.
• Visits can be scheduled 24 hours in advance with the activity director, and are subject to availability.
• When a visitor arrives, reception will screen the visitor to the same extent it screens all other persons that enter the facility.
If a visitor does not pass the screening, the visit cannot be conducted.
• Visitors will be required to wear proper PPE and will be required to comply with the facility’s policies and procedures in regard to social distancing, hand hygiene, infection control, and other COVID-19 related policies and procedures.
• If the visit is to take place in a designated visiting area, the resident will be escorted by facility staff to the designated visitation area and will be escorted to their room or other location, after the visit.
• Visitors may not walk around facility; they may only go to and from the designated visiting area, to ensure the safety of the facility’s residents and staff.
• Visitors must maintain a 6-foot distance during the visit, from both residents and staff subject to certain exceptions. If the resident to be visited has been fully vaccinated, there may be close contact. However, a well-fitting mask must be worn, and hand hygiene must be performed before and after the contact.
• If a visitor fails to comply with the facility’s visitation policy, or any policies or procedures of the facility, then the facility staff may end the visit.

Outdoor guidelines:
• Visiting hours are between 10:30 a.m., and 2 p.m. and 6 p.m.
• Outdoor visits will take place near the front entrance, weather permitting, and are strongly preferred over indoor visits.
• Visits will be limited to 30 minutes per visit, due to capacity required for social distancing.
• Visits can be scheduled 24 hours in advance with the activity director, and are subject to availability.
• When a visitor arrives, reception will screen the visitor to the same extent it screens all other persons that enter the facility.

PLEASE NOTE: We will make every effort to ensure visitation appointments are kept. Due to circumstances beyond our control, the facility is permitted to postpone or cancel visitation for the day depending on the weather or other unforeseen situations that may affect implementation of infection control protocol.
