Submission of this form indicates your request for NHS to send the PE Replacement Form via email to a parent/guardian at the provided email address for approval. Credit will not be granted if your parent/guardian does not submit the form sent to them. The PE Replacement form allows students grades 9-12 who have successfully participated in a recognized athletic activity with the option to replace one half credit (0.5 credit) of the physical education graduation requirement with an additional half credit (0.5 credit) in English, social studies, mathematics or science. It must be submitted during the season of the recognized athletic activity.
