Girls can earn a patch for Early Bird Registrations between April 15 - May 31, 2013. Leaders must request the patches using this form after renewals have been completed. This request form must be submitted before June 7, 2013 in order to receive incentives. Patches will be available for pick up in the Council shop after June 7th. (Please indicate if you are an outlying area and need the patches mailed to you.)
Registrations submitted to Council using Cookie Dough, Cookie Credit, or Opportunity Funds as payment should be included in this request.
For Troop Leaders**
Official Girl Scout Personalized Name tags will be given to Troop Leaders who register their troop during Early Bird Registration AND complete their Annual Troop Review.
If you are participating in our Individual Scout Pathway, please reference Troop 1 in the Troop number Field and Guardian's name in the Troop Leader field. Annual Troop reviews are not required for Individual scouts.
If you are a part of a Troop - PLEASE inform your Troop Leader that you have completed your renewals online. DO NOT COMPLETE this form
**Please be sure to check your Troop Roster in eBiz before submitting this request to ensure you are requesting enough patches for the girls.

Name tags will only be awarded to all 01 and 02’s who complete the following:
• Early Bird register, using the new online system or by submitting the appropriate forms
when using Cookie Credit, Cookie Dough, or Opportunity Funds.
• Register 50% or more of their "active" girls
• Complete the Annual Troop Review
