1. To get a copy of this form, visit our resources page and request it - learningarchitects.com and visit the Working Remotely section or the Resources section.
2. Delete this text block once you have made and renamed your own copy.
3. Change the cover image (the image is 1600x44 pixels).
4. Alter the questions as you wish. We chose the question 'Feeling organised?' as it gives a sense of how in control the student is. Of course, you'll know the students in your care to keep a closer eye on ;-)
5. Create a spreadsheet for the responses by clicking the Responses tab, then the little green icon 'create spreadsheet'. From there you might like to create some conditional formatting to highlight any responses that are low.
