Damage Appeal Form

Please use the form below to submit your damage appeal. If you are appealing multiple damages for different reasons, this form may need to be submitted more than once. The deadline to submit a completed appeal is 11:59pm CST on Wednesday, July 1, 2015. Appeals received on July 2 or later will not be considered. The appeal committee will complete the appeal process and, if necessary, make any adjustments to a student’s account in July. If you completed an express check-out, you are not eligible to submit an appeal as stated on the Express Check-out Form.

Include all your information below and make the appropriate selections to indicate your type of appeal. You should have your Room or Apartment Condition Inventory (found in mySLU under Tools>Student Housing) and student account bill available. If you experience any trouble with this form, please send your information, including your name, Banner ID, description of appeal, and rationale for appeal to reslife@slu.edu. You may also send copies of any supporting documentation (for example, emails or photos) by email to reslife@slu.edu or via the postal service (3744 W. Pine Mall Blvd., Brown Hall, St. Louis, MO 63108-3306), as you are not able to upload those documents into this form.

Please remember you will not meet in person with the Damage Appeals Committee, and the information you submit as part of your appeal will be the only information the Damage Appeals Committee will consider.

IMPORTANT: You should not wait until the appeal process has ended before paying your University account balance. You should pay your account balance when due to avoid finance charges. If your damages are reversed, you will not receive a credit for any finance charges incurred as a result of an unpaid balance.
