CogSci Code of Conduct report form
You may use this form to submit a report of a Code of Conduct violation if you are being harassed, notice that someone else is being harassed, or have other concerns.  You may make a report either personally or anonymously.  You may alternatively send a report by email directly to csscodeofconduct@gmail.com.  Any reports submitted using this form or by email will be accessible by the Governing-Board authorized Code of Conduct implementation team, which is comprised of Charles Kemp, Virginia de Sa and Penny Pexman.  You may also make a report in person to a member of this team.
