This form is for yours, ours, and the dog's protection while being rented. Please fill this out and sign it before renting your dog or you will not be allowed to complete the rental.

I do not own the dog, I am renting through Pups on the Plains.

I will keep the dog on leash at ALL times

Pups on the Plains is not responsible for any damages to my property or anyone else’s property or anyone else's property during the time of the rental

I am responsible for the general care and protection of the dog during the rental period which includes but is not limited to providing water and cool areas when necessary.

I am responsible for following all instructions given to me by the dog’s foster parent at the time of pickup

I am responsible for returning the dog at the time agreed upon, or before 10:oo pm (CST).

Pups on the Plains agrees to provide health certification of every animal upon emergency.

Pups on the Plains is providing a dog to be known of good temperament and health and I could be responsible for any changes in their health if it seems I was irresponsible with the dog

The responsibility of any injury to the dog during the rental period will be the joint responsibility of myself and Pups on the Plains.

Pups on the Plains is not responsible for any injury done to any persons or other animals during the rental by any Pups on the Plains canine.

Pups on the Plains cannot guarantee the health, behavior or actions of the rented dog.

You cannot bring said animal to the dog park, unless cleared previously with foster. Failure to follow this important rule will result in a fine.

If renting a dog known to be recovering from Heart-worms, I agree to not bring that dog on any hikes, to the dog park, or any long walks.

You may not bring the said animal until a household with other animals (dogs, cats) unless cleared with an OFFICER, not just the foster.

If renting a dog known to be recovering from Heart-worms, I will be ONLY hanging out with that dog indoors unless given explicit permission otherwise by a POP officer

I give permission to Pups on the Plains to use my photos, videos, etc. posted about or with the dog. As well as contact me by phone number or email at any time.

I agree to pay the full amount of my intended rental time 12 hours before picking up my rental dog

I agree that if I cancel less than 12 hours before my intended rental time, I will only be given half of a refund

Failure to comply with the rules given by the foster and Pups on the Plains can result in a penalty fee and/or the inability to rent a dog from Pups on the Plains. Purposeful harm done to the dog will result in charges pressed.
