Transcript/Records Request
Please complete the following information.  If you need transcripts or other records sent to more than 3 recipients, complete an additional form.

Please allow 10 BUSINESS DAYS for records to be processed. Please note that we are unable to process records requests during Fall Break, Thanksgiving Break, Winter Break, Spring Break, and for the last two weeks of June and first week of July.

Due to FERPA regulations, records will only be released at the request of former students or, if the student is currently under age 18, the legal guardian(s) on record.
