Having to submit an incident report is never a fun experience. First off, we are sorry you had to experience that! You are appreciated for your contributions as a referee! The following form has several sections. As you take the time to submit this incident report, please BE SPECIFIC in your answers. Assume that the committee reviewing the report does not know anything about the anything.
- Avoid abbreviations for schools/teams. e.g. PSS could mean Pemberton, Princeton, Penticton.
- Please try to get NAMES of coaches and players involved.
- Please do your best not to mix up home and away. (It is embarrassing when we submit a report to the wrong school!)
- Please be specific in the incident details section. We can condense things during the follow-up / investigation. It is difficult to follow up appropriately without sufficient details!
- We do not need details on free throw procedures, etc. after the fact.
Essentially, it is much easier for us to follow up and support you, if we do not need to reach out for additional information. Thank you for your assistance with this!
