The Light of Christ Catholic Collaborative Baptismal Form

Please read all of the below information before completing the form.

Form
• Please take care to complete this form accurately. Information from this form is used to create the Baptismal Certificate.

Confirmation Email
• Once form is received and processed, you will receive a confirmation email that the baptism has been scheduled.

Baptism
First and second Sunday of every month at both Parishes.
• Saint Bridget's Baptisms 1:30pm
• Holy Ghost Baptisms 12:00noon
• There are no baptisms on Holy Days (Christmas, Easter, etc.).

Baptism Prep Class
Your first child:
• Baptism class prior to Baptism
• Baptism Prep classes run every 4th Sunday of the month
Not your first child:
• Clergy will do a prep over the phone prior to Baptism

What are the requirements for Godparents?
• Only one male and one female Godparent
• at least sixteen years of age
• One Godparent must be a Confirmed and practicing Catholic
• Provided at least one Godparent is a qualified Catholic, the other may be a baptized member of a different Christian religion. Therefore, non-Christians (Jews, Muslims, etc.) May not serve as Christian Witnesses.
• We do not need any certificates or paperwork from either Godparent.

Do Godparents need to attend the Baptism Prep Class?
• Godparents are not required, but they are welcome.
• Only parents are required to attend.

I'm married, why is my maiden name on the Baptismal Certificate?
• The certificate is a legal document.
• Parent's names appear as they do on a birth certificate.

A Proxy stood in the for a Godparent, why isn't the Proxy on the Baptismal Certificate?
• A proxy's name will not be included.
• Only the Godparents’ names are included.

Your first child: Baptism Prep is held every fourth Sunday of the month following morning Mass. You will need to be added to the list for the class. Godparents are not required to attend. Please note any questions or scheduling issues.
