Obtainium Cup Contraptor's Rally - July 21, 2013
If you want to enter a vehicle or bicycle in the rally and receive a medal, you must fill out this form and pay the $25 application fee.

Please note the following:
• All entries must be registered by July 14 to receive a medal; hand delivered entries should be given directly to the judges at the start of the race
• Waiver must be signed by all team members
• If team member is under 18, they must have a waiver signed by a parent or guardian
• Costumed bicyclists need not have a vehicle or team name

All participants must also download and sign this waiver and release form, and mail to:
