All parents, students, and volunteers attending the 2021 Trunk-or-Treat event must fill out this form to attend or participate in this year's event.

Completion of this form verifies the attendee(s) acknowledgement and acceptance of the following waiver: COVID-19 WAIVER & AGREEMENT.

2021 SOLAR PREP GIRLS TRUNK-OR-TREAT | COVID-19 WAIVER & AGREEMENT
I attest that:
1) I am not experiencing any symptoms of illness such as a fever, cough, or shortness of breath.
2) I do not believe that I have been exposed to a person with a confirmed or suspected case of COVID-19.
3) I am aware that I must follow the safety and hygiene protocols that have been recommended by the CDC, Dallas ISD and Dallas County.
4) I understand and acknowledge the risks to myself and to my family members in participating in large group events, such as the case with the 2021 Solar Prep Girls Trunk-or-Treat and hold harmless Solar Preparatory School for Girls, Dallas Independent School District, Solar Prep Girls PTO, their sponsors, officers, agents, volunteers, and/or employees from any and all liability, claims, demands, actions and causes of action whatsoever while attending or participating in any activity at the 2021 Solar Prep Girls Trunk-or-Treat.
