This form is to be completed by Audiology staff only following confirmation of deafness / hearing loss
Please note, referrals can only be accepted if the following requirements have been met:
1. Child has an identified, diagnosed deafness / hearing loss
2. Evidence from an audiologist is available to support the diagnosis and/or history
3. Parents have been consulted and have consented to this referral
Upon receipt of the referral we will:
For babies diagnosed through the NHSP contact the family within 2 working days. For preschool children diagnosed outside of the NHSP programme contact will be within 5 working days term time only. Our referral process can be found on our website:

If the referral is accepted we will:
•
Enter the pupil onto our referral database
• Arrange an assessment visit or visits• Complete the BSCS assessment and provide a written BSCS new referral report within 8 working weeks including future support if required
The BSCS is funded via a joint arrangement between the Six Berkshire Local Authorities
