As consideration for the Student and his (or her) family being permitted to attend the Pittsburgh Japanese School and participate in activities associated therewith (the “Activities”), I (We) hereby acknowledge and agree to the following:
ピッツバーグ日本語補習校への通学、諸活動への参加にあたり下記につき同意致します。


1. I (We) recognize and understand that there are risks associated with the Activities, including but not limited to, bodily injury and damage to property.
補習校への通学、諸活動への参加においては、なんらかのリスクが伴うことを了解します。

2. I (We) declare that the Student's mental and physical conditions do not prevent the Student from performing the Activities.
園児・児童・生徒が補習校の活動に支障がない心身状況であることを宣言します。

3. I (We) agree to indemnify, defend and hold harmless Pittsburgh Japanese School and its teachers, officers, clerks and volunteers (the "Indemnified")from and against any and all claims, demands, losses, damages, causes of action, suits and liabilities for injury to or illness of the Student and his (or her) family or the visitor, or for loss or damage to any property, directly or indirectly arising in any way incidental to the Activities, even through caused by or arising from the active or passive, sole, joint or concurrent negligence of the Indemnified.
補習校での活動に関連しての園児・児童・生徒および保護者などの家族または訪問者のけが、病気、器物損傷につき直接、間接を問わず、補習校およびその教員、運営委員会、職員、ボランティア（関係者）の過失、責任を問わないことに同意します。

4. I (We) authorize the Indemnified to obtain medical assistance and/or treatment when deemed necessary by the Indemnified and agree to pay all costs associated with such medical assistance and/or treatment.
園児・児童・生徒および保護者などの家族、または訪問者の必要と思われる医療措置、治療については関係者に委託し、その全ての費用を負担します。

5.  I (We) will adhere to any additional condition(s) for attending Pittsburgh Japanese School.
補習校が就学のための条件を追加した場合は、それに従います。

6.    I (We) acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending Pittsburgh Japanese School and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Pittsburgh Japanese School may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Pittsburgh Japanese School's employees, volunteers, and program participants and their families.
新型コロナウイルスの高い感染力について認識し、補習校に登校することにより私あるいは子供が新型コロナウイルスに接触又は感染する可能性があり、ひいてはけが、病気、恒久的な障害、あるいは死亡につながる可能性があることについて認識します。補習校に登校することによる新型コロナウイルスへの感染リスクは、私自身あるいは補習校の職員、ボランティアなど関係者及びその家族も含む全員の行動、または不注意から発生するものであることを理解します。

7. I (We) recognize and understand that participation as a student, teacher, volunteer, parent, or visitor includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist.
園児・児童・生徒、講師、ボランティア、保護者、または訪問者として参加することでMRSA、インフルエンザ、COVID-19等の感染症にかかる可能性があることを理解しています。個人の努力やルールを守ることでリスクが低減されても、重症化・死亡のリスクは存在することを認識しています。
 
8. I (We) agree to comply with all current Centers for Disease Control (CDC), Pennsylvania Department of Health, and local health department recommended preventative actions, guidelines, terms and conditions for participation regarding protection against infectious diseases, including but not limited to social distancing, personal hygiene and use of a face mask. Failure to comply with such preventative actions may result in removal from the school and programs without refund.  
ソーシャルディスタンス、衛生管理やマスクの使用など、CDC（米国疾病予防管理センター）、ペンシルベニア州保険局、及びその他の保険局が勧める感染症の予防法や対策ガイドラインに従います。これらの予防法に従わない場合、返金なしに退学となる可能性があることを理解します。

9.  I (We), HEREBY RELEASE AND HOLD HARMLESS PITTSBURGH JAPANESE SCHOOL, its officers, officials, board, agents, and/or employees, other participants, sponsors and advertisers (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.  
過失による個人の病気、障害、死亡、あるいは所有物の危害、損失があった場合でも、法律で認められる最大限の範囲で、ピッツバーグ日本語補習校及びその役員、職員、委員、代理人、参加者、支援者や広告主に対し責任を問うことや損害を与えることをせず、何も請求しないことをここに宣言します。

10.  I (We), the undersigned below, understand, certify, and acknowledge that I have distributed and communicated to all of my family members the language of this form as well as CDC, Pennsylvania Department of Health and Allegheny County Health Department COVID-19 guidelines, requirements and suggestions.  
下記の署名をもって、家族全員にこの免責同意書の意図を伝えるとともに、CDC（米国疾病予防管理センター）、ペンシルベニア州保険局、及びその他の保険局が勧める感染症の予防法や対策ガイドラインに従う必要性についても家族全員に伝えることを保証します。

11.  I (We) and my family members KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation.  
上記のリスクを承知の上で、他人の過失から発生しうる既知／未知のリスクに関わらず、参加の責任を全て負うことに同意します。

12. I (We) understand that Japanese language summary translation herein is for my reference and convenience only and shall not affect in any manner the construction or interpretation of this Consent.
この同意書の日本語訳はあくまでも参考のものであり、この同意書の内容解釈になんらの影響を与えないことを理解致します。
